Thanks for the chords, will try these out today. Do you have any tips regarding the picking pattern? I'm finding it a bit hard to decipher to a solo guitar from the original.

There are two pluck instruments playing simultaneously which is why it's difficult to hear the picking style. Sufjan seems to be playing a basic alternating thumb pattern: 6th string (thumb), 2nd (finger), 5th (thumb), 3rd (finger), essentially alternating from the outer edges of the chord inward. He will also hammer a few strings: for instance, he most plays the D only on the 2nd 3rd and 4th strings, alternating on 3rd/4th with his thumb, and when he hits the 3rd string he's hammering quickly from 0h2 every other time.
Also, OP, I believe you're mistaken about the A chord (the important chord you mention). I believe he's walking down the 5th string on an A7sus4...

Also, I watched a live video on youtube... he's really playing with a capo on 3. On the bridge he has some interesting shapes up the neck, suggesting that the chords there are variants rather than standards written here. Maybe it's worth studying more closely. I do like capo on 8 because it allows you to improv the notes from the second instrument.
And it might be nice to arrange the 3rd chorus to hit the high notes, maybe something like this:
