Similar to what "helpweresunk" said, the verse is tabbed correctly, but the chorus goes up to 10 then 12 on the D string, or 3 to 5 on the A string. Also, the key change is 1/2 step up, so for the final chorus your 3 5 10 12 pattern becomes 4 6 11 13 or 4 6 A4 A6. Sorry I didn't actually tab that out, but you all are smart people!

You're missing the key change at the end and the chords are definitely wrong. The first two (the 3s and 7s) are right, but then it goes (I think) 10 and 12, just going further up the neck.
