The chords you need are:
C - Am7 - G - Em - F - G

You will only have to take your ring finer of the 5th string to go from the C to the Am7 chord
 
 
This is the order and how often you'll have to play them:
 
C - C - Am7 - Am7 - G - G - Em - F - G
 
 
This is the picking pattern:

So it will be like this:
