[Verse]

C
I've been thinking about
                  Em
The way the world turns
              Am
And my stomach churns
                C
When it finally hits me out of the sky
                  Em
I knew this day would come
                  Am
I'm on the front line

G Am

[Bridge]

      Am G
Oh I don't know how to take it in
               Am
Is love just suffering?
                               G
Cos I can see where the chapter ends

[Chorus]

         F
I've got autumn leaves
         F C
And heartbreak dreams inside

                Am C
With just my shadow following me
                     Em
Out into the cold, as I walk past the trees
               Am
To look for my love

G Am

[Bridge]

I believe the C/B, Am/G, and D/F# chord are played like so: C/B = X22010 Am/G = 302210 D/F# = 200232 The chords themselves are technically called Bmaj7/Bb, B6(Abm7)/Gb, Db/F since it's tuned down a half step.

Good basics, but a few more details: In the Em he transition to the A with Esus4. The Am becomes Am7. The G is really G6, becoming G7 right on "I've got" and the F is really Fmaj7. These differences will help with the notes you sing and give it that more mellow, beautiful sound.
