this is my transcription (r) of it, probably with its flaws, but here you go!!!
 
p.s.: a B#dim is the same as a Cdim (as well as F#dim, Adim and D#dim),
but because after that diminished chord there comes a C#m and it's not
logical to have two C's in a key.
