I think the C#m with the decending root sounds right, but I'm not sure.

I think that C -> B on "journal-oust" should be a C -> D/A instead (a D major chord with A as bass note).

That Cm/C is a bit off; G#/C (or if you want to get technical, G#/B#) sounds better. Also, the song ends on Fmaj7 - E (starting on the word "feedback").
