After the bridge when the key changes there are some problems with the chords, namely C#m. It would be helpful to post your own numbering of what you think the chord should be as sometimes this site automatically illustrates the chords and it may not be what you intended. Otherwise the song is very nice and your transcription is good up until that point.

After the bridge, it is a Cm, not a C#m.

Yes, no matter how you play it, the progression calls for Cm following the bridge, otherwise, you are modulating 1 step on the rest of the progression, but two steps up on that one chord. Thanks, carrie63.
