I think he might actually be lifting his middle finger in between every measure. i.e for strum pattern (D - D - U D) lifting middle finger on the (U D). On transition to new chord, either Fj7/C or C/G the I add an extra D to the strum pattern and only lift middle finger on that final D strum.
