I was messing around with the partial chords and found that the first chord with the hammering-on is definitely 0 on B, 0 on G to 3 on B, 0 on G. Just try it out, it sounds exactly like the song. Also, the baseline continues going throughout the outro, so you've gotta have the 3 on A going in between each partial chord. Hope some of this helps, let me know how it works out for you.
